These are the basic outline to improve the community that empowers the youth in White Plains community. The overall performance of White Plains will have a basis to have a sustainable community organizing and planning that improves the social, political, economic, and cultural values across this urban area in New York State.

White Plains is an urban center in Westchester County, New York State. The city is the center for economic and industrial hub of the entire country. The thriving city is a suburb of New York City, which is home to more than 20 million people, which is the center of the country’s economy. As part of the community organizing and planning, the census of the city as of 2010 indicates that the total population is 58,459. The racial breakdown shows that there are at least 48.9% whites, 29.6% Hispanic, Native Americans with 6.3%, 13.2% Black, less than 0.1% and Pacific Islander of 0.3%. The median income for every household in the city is $58,545. In terms of health teaching activities, there are numerous facilities that you can apply this program. One obvious location is hospital while others are in clinics, schools, and areas that are frequented by people such as in malls or government centers.

Community Analysis: The community based health strengths are the presence of tertiary hospitals that are scattered across the center. There are private clinics in the area that provides an accessible health care system for patients and communities.  White Plains implements an intensified campaign to promote health awareness by the City Health Department. Both private and public health institutions are cooperating with each other to lower down mortality and morbidity rates across the city. Health promoting recreational centers such as gym, spa, and beauty centers is also present in the city to improve the health and wellness of all residents.

Community-based health risks: White Plains, New York is no stranger to communicable diseases. During winter, the city experiences a fluctuation of respiratory diseases because residents are exposed to an extreme cold environment brought about by the snowstorm. Since the city is an urbanized area, food-borne diseases are also risking residents in restaurants and other public places. Sexually transmitted infections are also risking adults who are exposed to unprotected intercourse that usually happens at public recreational centers. Since there are also structures that did not pass safety and fire certificates, the risk of accident could result into physical injuries that will result into complications.

Health problems in the city are hampered by several life-threatening diseases. Heart diseases or other diseases of the circulatory system are the main life threatening diseases. Neoplasms or cancer is the second leading morbidity and mortality cases in the city. This is caused by lack of engaging in healthy diet, exercise, and not checking their health status. As for the sexually transmitted infection, fungal infections such as chlamydia are the leading morbidity case while gonorrhea is the second leading sexually transmitted infection. This is brought about by engaging in unprotected sex for the residents having affair with multiple partners.

Nursing diagnosis 1: Risk of developing circulatory diseases related to eating unhealthy foods as evidenced by heart attacks, angina pectoris, and cardiovascular accidents. The community-based health risks and problems that will be used in this teaching plan are about the significance of making a follow-up check up with the physician regularly. There is a big risk that a patient can suffer the similar fate if they fail to comply with the advice of their physician. Complying medical regimen will minimize the risk of complications brought about by the consequences of heart ailments.

Nursing diagnosis 2: This is body weight disturbance as related to lack of exercise as manifested by obesity. As a community-based health risks and problems is about the growing issue of obesity. This is usually affecting adults starting from 25 years old and above because the body’s metabolism is slowly going down after the patient continues to stay in an unproductive lifestyle. Obesity can cause numerous life-threatening disorders, which could hamper the lifespan of the patient. One example is developing diabetes that is one of the contributing factors leading to other diseases such as circulatory diseases in the body.

Nursing Diagnosis 3: This is the Risk of Sexually Transmitted Infection as related to unprotected sexual contact as manifested by acquiring fungal and chlamydia infections. Sexually transmitted infection or STI is one of the growing problems among the adults. Most infected patients have been partying when they meet random strangers and then engage in sexual intercourse. When engaging in unprotected intercourse, patients are at risk for developing a variety of communicable diseases because there are other infected individuals who were not aware about their infection that could transfer it to other individuals. For as long as there is no protection, the risk is the main problem for vulnerable patients.

Community Health Teaching Plan

Aggregate: The aggregate concern for White Plain is about the prevalence of life-threatening diseases affecting the residents of this city. Another aggregate is to stop the increase of sexually transmitted disease across White Plains or to minimize the cases if stopping the spread of infection is not contained to the residents. This is because as an aggregate, the data shows that residents who are at risk are usually adults because they are engaged in unhealthy practices that are causing their health to acquire infection. Heart diseases and cardiovascular disorders are just some of the pertinent facts that impacts the overall health and wellness of individuals suffering from a life threatening diseases. Residents who acquired or developed these diseases could risk their longevity because the causative agents are slowly threatening their health and wellness that might risk their safety while they are not seeking for medical help according to Dietscher, et al., (2014).

Objectives: Health teaching is the main focus that will be applied for the residents of White Plains in New York State as well as the visitors in the area. The goal of health teaching is to increase awareness regarding the prevalence rate of diseases in the city of White Plains. This is a significant issue because it allows residents to have the opportunity to learn more about these medical conditions that will let them start cooperating with the health care institutions and private companies who wants to support the health care system of White Plains. The plan for health teaching is a long-term goal for every resident in the area so that they can protect themselves from either developing or acquiring the disease. If they are going to learn and understand about the implications of health threats and risks in their area, residents will start cooperating with both private and public institutions as mentioned by Powers, et al (2010).

Plan: The main plan is to apply health teaching to all public and private institutions across the White Plains. This is under the initiative of the health care department of the city of White Plains. The main theme is to discuss certain type of diseases such as heart ailments that are causing a significant number of mortality and morbidity. One example is discussing Angina Pectoris, which is a medical condition wherein the coronary arteries are slowly blocked by hardened fatty deposits along the walls of the arteries. Patients suffering from angina pectoris starts to suffer from chest pain, dizziness, and shortness of breath because their heart is now affected by the blockage made by the hardened fatty substances along the walls of the coronary arteries. If the condition worsens, the patient may eventually suffer from an enlarged myocardium, which could lead to heart attack.

As an implication, it has been learned that health teaching is an important measure to inform the public regarding the importance of understanding about health diseases. Learning helps the residents and visitors of White Plains to be more careful with their health. This is because being unaware about the consequences of communicable and non-communicable disease could risk their health and safety, especially if they will be infected with life-threatening ailments. Compliance with the medication regimen is another important aspect that can be brought about by health teaching. The facilitators will provide reading materials for the patients to ensure that their knowledge becomes accessible with the plight of knowledge being applied to them to save their life from developing diseases. Health teaching is provided on a consistent basis because it helps individuals to improve their lifestyle by adapting health practices to improve their health. For this result, it will decrease the number of mortality and morbidity in the future according to Pleasant, (2011).
