RE: Review of Academic Assessment

Dear (Dean’s surname),

I would like to formally request for the review of my academic assessment in Biochemistry as I have received an F for the said subject. Throughout the entire semester, I have exerted great efforts to meet all the requirements in the said subject. I usually dedicate two hours a day to prepare for my classes. In the same manner, I also prepared extensively for the exam periods, dedicating at least six hours a day to the review of my lessons. As such, my grade in biochemistry was considerably fair before the final examinations therein.

The feedback provided has shown that my failure in the said subject has been largely attributable to my failure in the final examinations. Admittedly, I was not able to prepare extensively for this examination in view of my father’s accident. My father’s accident has tremendously affected my emotional health. The constant anxiety with respect to his well-being has resulted to my depression which in turn prevented me from preparing for the examination in Biochemistry. 

The review of my academic assessment in Biochemistry is of paramount importance to my goals and objectives. As a child, I have dreamt of becoming a dentist. Hence, a better mark in Biochemistry would allow me to continue on to the study of dentistry. I believe that my study habits and goals have already changed as compared to the first time I started studying here. Back then, I used to skip class and experience a difficulty in time management. However, with a clearer objective (that is, to get into dentistry), I have developed the discipline needed to fulfill my dreams.

It is in line with these circumstances that I am humbly asking for the review of my academic assessment in Biochemistry. Indeed, such unfortunate event has greatly affected my performance in my final examination thereby resulting to my failure in the subject.

RE: Application for Exemption from Credit Hour Limit

Dear (Dean’s surname),

This is to formally request for an exemption to the credit hour limit for the coming semester. I enrolled in four subjects last semester. Unfortunately, I did not pass all of the four classes that I have enrolled in. Aside from this, my current academic condition also limits my maximum course load to four courses per semester. In view thereof, I may no longer be able to keep up with the minimum requirement of course load for dentistry (at least eight courses a year). As such, I would like to petition for the increase in my credit hours beyond the maximum to accommodate another subject. I believe that such addition would allow me to obtain all the courses necessary to help me get into dentistry by next year. 

I have made various preparations to help me cope with a heavier load this semester. First, I have developed a study table to help me study properly for all my subjects. I also intend to take one science course to give more time to my other courses. Also, I intend to add first year microeconomy as a distance program. Hence, I have already borrowed the required book from a friend to help me prepare for the rigors of such subject. In addition, my advanced readings therein reveal that microeconomy is basically related to macroeconomy, a subject which I have already accomplished. Hence, this could once again help me in coping with a heavier schedule associated with the increase in my course load.

Getting into the dentistry program is one of my immediate goals for the next semester. As such, it is of paramount importance that I obtain all the required number of courses by the end of this semester. It is in line with this that I would like to humbly request for the increase in my credit hours beyond the maximum to accommodate another subject (Microeconomy). Rest assured, I have developed various steps by which I can cope with the demands of such increase in my work load.
